Does anyone know why sometimes you will open a chest from something you killed, then see an item disappear from it a split second after opening it?

For x reason and item made it past the level range for that chest and was removed.

Ahh, that makes sense. It was in Umgah but I imagine every place has a max item level. I have even seen it happen on little Shal. Thanks for the clarification
